I release Gori Voice Studio, LLC, its instructors, independent contractors, and all other associates from liability for harm, theft, or injury that may be suffered by me and/or members of my family traveling to or from or during participation in activities and programs sponsored by this organization.
I hereby acknowledge that I am voluntarily assuming full responsibility for all risks of physical injury arising out of active participation in one-to-one and group vocal study and related activities.
I acknowledge and voluntary assume the risk that I and/or members of my household may be exposed to or infected by an infectious disease by attending and participating, and that such exposure or infection may result in personal injury, illness, permanent disability, and death.
I understand that the risk of becoming exposed to or infected by an infectious disease may result from the actions, omissions, or negligence of myself and others, including, but not limited to, instructors, independent contractors, volunteers, and program participants and their families.
I acknowledge that Gori Voice Studios, LLC has made available virtual instruction for me/my child(ren), that I may request virtual instruction in lieu of in-person instruction for any one-to-one lesson up to 15 minutes before the scheduled start of the lesson, and further, that it is my responsibility to do so if I, my child(ren) or any member of our household has an active case of any communicable illness.
On my behalf, and on behalf of my child(ren), I hereby release, covenant not to sue, discharge, and hold harmless Gori Voice Studios, LLC, its owners, instructors, and independent contractors of and from the Claims, including all liabilities, claims, actions, damages, costs, or expenses arising from or relating thereto.
I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Gori Voice Studios, LLC, its owners, instructors, agents, or representatives, whether infection by any infectious disease occurs before, during, or after participation in any program.
